RTMLink, LLC ("RTMLink," "we," "our") operates a Remote Therapeutic Monitoring (RTM) platform that enables physical therapy clinics to send daily symptom surveys and exercise program check-ins to their patients via SMS and email. This Privacy Policy describes how we collect, use, and protect your information.

1. Information We Collect

When your healthcare provider enrolls you in their RTM program through RTMLink, we may collect:

• Contact information: name, phone number, email address
• Health information: survey responses about symptoms (pain levels, mobility, function), exercise completion data, and related clinical metrics
• Technical data: device type, browser, IP address, and interaction timestamps when you access the survey portal
• Communication records: delivery status of SMS/email messages sent to you

2. How We Use Your Information

• Deliver daily symptom surveys and exercise program reminders via SMS or email
• Present your responses to your healthcare provider for clinical decision-making
• Track message delivery and system performance
• Comply with healthcare regulations and billing requirements

3. HIPAA Compliance

RTMLink operates as a Business Associate under the Health Insurance Portability and Accountability Act (HIPAA). We maintain a signed Business Associate Agreement (BAA) with each healthcare provider using our platform. Your Protected Health Information (PHI) is handled in accordance with HIPAA's Privacy, Security, and Breach Notification Rules.

4. Information Sharing

We do not sell, rent, or share your personal information with third parties for marketing purposes. We share information only with:

• Your healthcare provider (the clinic that enrolled you) for clinical care purposes
• Service providers who assist in delivering our platform (e.g., Twilio for SMS delivery, SendGrid for email) under strict data processing agreements
• Legal authorities when required by law or to protect rights and safety

5. Data Security

We implement industry-standard security measures including encryption in transit (TLS) and at rest, access controls, audit logging, and regular security assessments to protect your information.

6. Data Retention

We retain your information for as long as your healthcare provider maintains your enrollment in their RTM program, plus any additional period required by healthcare record retention laws (typically 6-10 years depending on state). You may request deletion by contacting your healthcare provider.

7. Your Rights

You have the right to:

• Access the health information we hold about you (via your healthcare provider)
• Request corrections to inaccurate information
• Opt out of SMS communications at any time by replying STOP
• Request deletion of your data (subject to legal retention requirements)

8. SMS Communications

When enrolled in an RTM program, you may receive up to 1 SMS message per day containing a link to your daily survey. Message and data rates may apply. You may opt out at any time by replying STOP. Reply HELP for assistance. See our SMS Terms for full details.
